Informed Consent and Acknowledgement

I hereby give my approval for my child’s participation in any and all activities prepared by Halls Community Park and/or Hard Knox Flag League, without limitation. In exchange for the acceptance of said child’s candidacy by Halls Community Park,

I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless Halls Community Park and/or Hard Knox Flag League, and all of ts respective officers, agents, and representatives from any and all liability for injuries to said child, myself and all others, arising out of or in association with my participation, attendance or otherwise activities at Halls Community Park and/or Hard Knox Flag League, or otherwise, and without limitation.

In case of injury to said child, I hereby waive all claims against Halls Community Park and/or Hard Knox Flag League, including all coaches and affiliates, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event. There is a risk of being injured that is inherent in all sports activities, including flag football. Some of these injuries include, but are not limited to, the risk of fractures, paralysis, or death.

Medical Release and Authorization

As Parent and/or Guardian of the named athlete, I hereby authorize the diagnosis and treatment by a qualified and licensed medical professional, of the minor child, in the event of a medical emergency, which in the opinion of the attending medical professional, requires immediate attention to prevent further endangerment of the minor’s life, physical disfigurement, physical impairment, or other undue pain, suffering or discomfort, if delayed.

Permission is hereby granted to the attending physician to proceed with any medical or minor surgical treatment, x-ray examination and immunizations for the named athlete. In the event of an emergency arising out of serious illness, the need for major surgery, or significant accidental injury, I understand that every attempt will be made by the attending physician to contact me in the most expeditious way possible. This authorization is granted only after a reasonable effort has been made to reach me.

Permission is also granted to Halls Community Park and its affiliates including Directors, Coaches, and Team Parents to provide the needed emergency treatment prior to the child’s admission to the medical facility.

Release authorized on the dates and/or duration of the registered season.

This release is authorized and executed of my own free will, with the sole purpose of authorizing medical treatment under emergency circumstances, for the protection of life and limb of the named minor child, in my absence.

Important League Information

Please note the following important pieces of information:

1.  Your registration fee covers the cost of participation, a jersey and ancillary team / league equipment, alone. 

2.  Parents / Guardians remain solely responsible for all other necessary clothing, equipement, footwear and otherwise. This includes any additional items / costs, such as individual trophies, banquets, admission fees and all which is otherwise not listed previously, and above. 

3.  Halls Community Park and Hard Knox Flag League hold the exclusive rights to refuse the registration or participation of any player, team, coach, parent, guardian, or otherwise, and at its sole discretion, without either warning or reason. 

4.  NO REFUNDS WILL BE GIVEN, UNDER ANY CONDITION, OR FOR ANY REASON, WHATSOEVER.

5.  THERE MAY BE EVENTS WHICH REQUIRE A MODEST GATE FEE, AND WHICH ALL ATTENDEES ARE REQUIRED TO PAY, AT THE TIME OF ENTRY.  THESE FEES DO NOT APPLY TO COACHES, PLAYERS, AND OTHER LEAGUE VOLUNTEERS. EVENTS REQUIRING A GATE FEE WILL BE ANNOUNCED IN ADVANCE, IF ANY ARE HELD. 

6. Please note that the listing of any preferences (e.g. jersey number, team / coach, pairing of players on single team) are for informational purposes only, and that there is no guarantee that such can be honored, in all cases, or in any event, including preferences related to jersey number, coach or in the pairing of players.  

Code of Conduct for Players, Coaches and Observers

Our League is fortunate to enjoy highly-competitive games amongst equally-matched players, passionate coaches, the electric atmosphere of an enthusiastic crowd, and where the will to achieve, and to win, can be great.  We not only encourage this identity, but embrace it.

But first and foremost, the core tenants of the Hard Knox Flag League should be unwaveringly clear, and often reminded.  They are:

Safety

Sportsmanship

Fundamentals

Education

Fair play

Teamwork

Individual Effort and

Respect - for the game, and all involved with it

We will advance, encourage and insist upon these tenants at every opportunity - and we will be swift to remind and unyielding in their enforcement, whenever necessary.   

With that in mind, the following are our expectations of every player, coach, official, fan or observer.

1.  The opponent should be treated always be treated cordially, and with respect.

2.  Officials should be recognized as impartial arbitrators who are trained to do their job with the best of their ability.

3.  Familiarity with the current rules of the game and the recognition of the necessity for a fair contest are essential.

4.  Sportsmanship requires one to understand his or her own bias and the ability to prevent the desire to win from overcoming rational behavior.

5.  Applause for an opponent’s good performance is a demonstration of generosity and good will and should not be looked at negatively. 

6.  Your enthusiasm as a spectator includes a vital responsibility for good sportsmanship. Your habits and reactions determine the quality of sportsmanship, which not only reflects upon yourself, but your team and our League - but which is the single best and most important example to your child, and all others.

7.  Respect, cooperate and respond to coaches, officals, and League / Park personnel.

8.  Respect Park property and authority.

9.  Show respect for players who are injured.

10. Respect the judgement and strategy of the coach (even if you disagree).

11. Respect the judgement of game officials (even if you disagree).

12. Avoid profane language and obnoxious behavior at all times.

13. Avoid applauding errors or penalties of the opponents.

14. Refrain from heckling, jeering or distracting opponents - at any time, before, during or after a game or play.

15. Refrain from being critical of players, coaches or officials for a loss.

16. Refrain from entering or throwing objects on the playing area or in the bleachers.

17. Refrain from stomping of bleachers or the use of artificial noisemakers.

18. Refrain from using cheers that taunt or ridicule opposing players, coaches, cheerleaders or spectators.

19. Refrain from booing or showing displeasure with game officials or game activities.

20. All final decisions, rulings, findings, determinations and instructions are left to the Hard Knox Flag League Commissioner, alone. 



Your submitted registration, whether electronic or in hard copy format, will serve as proof that you have read, understood, acknowledge, and voluntarily agree and consent to all of the information, here - AND DO YOU UNDERSTAND AND ACKNOWLEDGE THAT A FAILURE TO ADHERE TO THESE RULES MAY RESULT IN YOUR BEING SANCTIONED, SUSPENDED, EJECTED, REMOVED, BARRED OR A FORFEITURE OF ANY PRIVELIGE TO ATTEND OR PARTICIPATE IN LEAGUE EVENTS, EITHER IN THE CURRENT OR FUTURE SEASONS, AND TO ENTER ANY AREA WHERE LEAGUE-SPONSORED ACTIVITIES MAY BE HELD? FURTHER, BY SUBMITTING THIS FORM, I AM DELIVERING AN ELECTRONIC SIGNATURE THAT WILL HAVE THE SAME EFFECT AS AN ORIGINAL MANUAL PAPER SIGNATURE. THE ELECTRONIC SIGNATURE WILL BE EQUALLY AS BINDING AS AN ORIGINAL MANUAL PAPER SIGNATURE.
